Basic Information

This block contains fields for honorific, name, title, institution, college or school, department, street address, city/state/zip code, country, telephone, fax, e-mail address, and personal Web page.

Please enter your information as you would like it to appear on a printed form or grant application. Use upper-case letters where appropriate. Examples:

"John Doe", not "john doe"

Select the appropriate rank from the picklist. If the list does not contain an exact match, pick the closest match.

Ethnicity, Gender, Citizenship Status, Disability?

There are separate fields for up to three academic degrees, plus a field for other education or training (internships, residencies, postdocs, etc.).

Although no particular order is required, we suggest that you list your degrees in the chronological order in which they were acquired, starting with the earliest.

Professional Experience (Job History)

Please list your professional positions in reverse chronological order, starting with the most recent and working backwards. There are separate fields for up to five professional positions. There is a general text area for other positions or job experience.

Publications

Again, please list your publications in reverse chronological order, beginning with the most recent. There are separate fields for your five most recent or representative publications. List other publications in the "Other Publications" text area. There is no limit on the number of publications that may be listed in this area.

Memberships/Associations

Self-explanatory.

Advisees

Self-explanatory.

Collaborators

Please include the collaborators' institutional affiliations and the nature of the collaboration (grant proposal, journal article, etc.).

Current and Pending Support

List current or pending grants.

Synergistic Activities

This field is required by the National Science Foundation (NSF). Here is the NSF's definition of "synergistic activities":

"A list of up to five examples that demonstrate the broader impact of the individual's professional and scholarly activities that focus on the integration and transfer of knowledge as well as its creation. Examples could include, among others: innovations in teaching and training (e.g., development of curricular materials and pedagogical methods); contributions to the science of learning; development and/or refinement of research tools; computation methodologies, and algorithms for problem-solving; development of databases to support research and education; broadening the participation of groups underrepresented in science, mathematics, engineering and technology; and service to the scientific and engineering community outside of the individual's immediate organization."
